Brahma, the maker of the universe, is delineated as having four heads, however, initially he had five. 

While making the universe Brahma made an excellent lady and Brahma with his four head named her Satarupa. In any case, Brahma was changed by his own creation. He had at no other time seen such a fine stunner. To empower himself to see her from all edges he allowed himself a second, third, and fourth head. Any place Satarupa went she discovered Brahma's eyes gazing at her. 

Disturbed, she rose skywards and so as to see her, Brahma made a fifth head for himself. 

Master Shiva, the destroyer, was observing this, and Brahma's activities extraordinarily rankled him. He felt that as Brahma had made Satarupa, she resembled his girl and it was his obligation to ensure her. 

He felt that it wasn't right on Brahma's part to be enamored with her and to show him a thing or two Shiva struck off Brahma's fifth head.
